Your completed claim form, including all necessary supporting documentation including your policy number, which can be found within your Certificate of Currency and your SHCS incident reference number, should be sent to:

Once your claim form has been received, your (brokers name here) Claims Advisor will verify that:
Once verified, your (brokers name here) Claims Advisor will review your claim form and supporting documentation to ensure you have provided all the information necessary to lodge the claim on your behalf.
Your claim will be lodged with (insurance name) here claims team who will review the claim in conjunction with the relevant policy and then determine if indemnity is to be accepted or not.
Once determined, your (brokers name here) Claims Advisor will liaise with the insured HCP directly and the applicable deductible will need to be paid by the claimant if they wish to proceed with the claim.
